Right off the top of my head, a taco comes to mind. You have the grain in the tortilla, the meat product in the ground beef, the dairy in the shredded cheese sprinkled on the top, the veggies in the shredded lettuce topper, the fat/oil is in the meat and for a fruit, this may be stretching it but since a tomato is technically a fruit, then chopped tomato on top would take care of the fruit aspect. Should you make a meatless one, then beans could be the protein source.

Another thing that comes to mind is carrot cake - the carrots of course are the veggies, the fat is the oil put into the batter. The grain would be the flour component and the protein would be the eggs. The dairy would be the cream cheese icing and so far as the fruit is concerned, some recipes call for crushed pineapple! Good luck!

broccoli, rice, chicken & cheese casserole.

why not a flat bread? You can buy pre-made frozen dough, so it'll help out a ton, so you don't have to wait for the yeast bread to proof.

You can go crazy with variations!

Go to allrecipes.com and search for a recipe by ingredients.

Some kind of soup, casserole or stew could have all 4.

the pyramid is orange - grains
green - vegetables
red - fruits
yellow - fats and oils
blue - milk and dairy products
purple - meat, beans, fish, and nuts [ I think you should get to cooking my friend

A Pot Pie or Shepards Pie would fit the bill...

Go to http://www.foodtv.com and search Shepards Pie.

It has EVERYTHING!

Ingredients:
.5lb ground beef
.5tsp dry mustard
.25tsp chili powder
1 small onion, finely chopped
.5 medium green pepper, finely chopped
16 ounces mashed refried beans
8 ounces tomato sauce
1.25 ounce taco seasoning mix

sour cream
finely shredded lettuce
shredded cheddar cheese
corn chips

Instructions:
cook and stir ground beef until brown; drain.
stir in mustard, chili powder, onion green pepper, beans, tomato sauce and seasoning mix.
heat to boiling, stirring constantly.
spread in ungreased serving dish.
spread sour cream over ground beef mixture. sprinkle with lettuce and cheese.
serve with corn chips.

takes about 25 minutes, maybe 20 if you have pre-shredded ingredients. this has been a favourite for and my kids for years; we make it 2-3 times per month, or for parties.
