The tomato juice isn't much different than water. Apply heat to water, temperature of water goes up.

Heres a tip: If the food is so hot that it burns your mouth, then let it cool for a bit.

In the future, you can ask the people making your sandwich to place the tomatoes on the bottom, under the meat. This might reduce the temperature of the tomatoes, or ask them to simply place the tomatoes on the sandwich after it comes out of the toaster.
