Yes, milk will soften the chocolate. Better to just throw it out, or eat it and start over again. Heating chocolate too much will take it out of "temper" and turn into a big globby mess.

I don't know what you are making, but if you can heat the chocolate mix for a while at low temperature maybe you can evaporate some of the water from the mixure.

kind of hard to tell if you dont say what it is you're making and the result you want to reach...

Yes that helps but when I'm melting chocolate I also add butter that helps a lot when I do it but depends what kind of chocolate you're using & anything else you put in there.
