Here's a fantastic tip!.!.!.the Viva paper towel trick!.
Wait fifteen minutes or longer until the frosting does not stick to your fingers when you touch it (it has to be a "crusting" icing!.!.!.others won't work), then lay a Viva paper towel (no design on it), and rub it lightly with your hands to smooth the icing!.
If you can't get Viva, waxed paper or parchment paper will do as well!. So will plain computer paper!.
I decorate cakes as a side business, and this is how I get the icing "smooooooooooooooth"!Www@FoodAQ@Com

Use a cake decorating frosting spatula !.!.!. then dip your (clean) fingers into water and smooth it down all over:) -very gently!.!.!.!. Another hint: Put strips of wax paper under the bottom edges of the cake, and just pull them out when you're done frosting the cake!. That will keep the cake platter clean from dripsWww@FoodAQ@Com
Well first, is your cake flat, I always level my cakes!. Second, what temperature is the knife, frosting and cake!? The cake should be room temperature, the knife cold, and the frosting slightly cold but still very pliable, like whipped cream or soft margerine in a tub, sometimes when you buy frosting from the store in those little plastic tubs, the ingrediants are not smooth enough to really get it pro, but theirs another niffty trick for that I'll say later!.
Do you have the right knife!? I mean something bigger than a butterknife and much flatter, butter knives are bad!
A detailed trick is to first put a crumb layer, thin quick layer of frosting to seal the crumbs in, shouldn't be very thick!. Then wait and add the real layer of frosting!.
Quicky trick, works with frosting like the store type!. After trying your best with the frost let it sit in the frig for a bit, not too long maybe five to ten minutes!. Then take it out and pat the edges in, without moving the knife much just touch them in to smooth them!. Sometimes this works other times it doesn't!.Www@FoodAQ@Com

These answers are great!.
If you use the paper towel method (or wax or parchment paper) to smooth the cake, you must make sure your frosting will crust, or dry at the surface first, so the towel or paper doesn't stick to the frosting!. Recipes of frosting made with all vegetable shortening or half shortening and half butter will typically do this!. Canned frosting from the grocery store or frostings made with all butter typically will not do this!. Once the cake is frosted and the frosting has crusted, place the paper towel (or paper) onto the cake and use light pressure with your hand moving in a back and forth motion to smooth the cake!. Reposition the towel or paper and repeat until all parts of cake have been smoothed!.
Alternately, you can use the spatula held under warm running water!.!.!.!.!.!.shake extra water off and use the warm moist spatula to smooth over the ridges in the frosting!. This will require repetition!. It works best with a long (13 inches or more) spatula, preferably with an offset handle!.
If you take a Wilton Course 1 cake decorating class, you will learn this (smoothing the cake) and many other helpful tips (how to bake cakes with even tops, grease the pans successfully, etc!.) in the first lesson!
