Depends on the cake

A layer cake gets cream in the middle by creaming the bottom layer and putting the top layer on

Individual cakes are typically filled with a pastry bag fitted with a tip that gets inserted into the bottom of the cake and cream is squeezed in. Depending on the size of the cake, there may be 1, 2, or 3 points of entry along the length of the bottom of the cake.

I mix up my cream fillings then fill me cheeks up with it, poke a hole in the cupcake with a pencil and just blow the stuff inside. I used to be able to do this very well but my years of smoking have caught up with me.

Two methods:
One is to pipe it in via the bottom of the cake going around and around till complete.
Two is to hollow out a tunnel, fill, then crumble the cake back on top before putting the top back on.

You make layers and put the cream filling in between the layers.
