Answers:

i watched a documentary about this on travel channel. i didn't hear there were bugs in ketchup but the coloring to make some dyes are made from bugs. not that there's bugs in it but some company's use certain types of chemicals found in bugs to make the dye.

There's bugs and unpleasant things in just about everything. Thing is, there is such a teeny tiny fraction of an amount, so it doesn't matter. You can't tell it is there, and couldn't detect it if there was a test for it. So don't worry, the salt in the ketchup is probably going to kill you before the bugs.

Yes, and yes. Bugs are in all we eat, THAT IS HARVESTED. But only a certain amount is allowed, and a lot gets sorted out through processing. From bread to Hershey bars, inside are little parts of bugs...They use food dye in some juices to make it look fresher, too.
