There is a recipe on the boxes of powdered sugar for frosting that is made by hand.
Take one box 1pound of powdered sugar and dump about 1/2 of it into carefully into a large bowl.
Then add 1teaspoon vanilla, 1 tablespoon butter or margarine and 3 tablespoon milk.
and blend carefully you may need to add more milk if you live in a dry climate or more powdered sugar if the humidity is high. This recipe is an art rather than a science. You can make in the consistency of a glaze or much thicker.
For chocolate frosting add 2 or 3 tablespoons of cocoa
You can also add food coloring and flavors like peppermint or almond.
The "miracle fudge" recipe on the marshmellow whip jar also makes a fantastic chocolate frosting. Just cook it one minute less and spread on you cake or cupcakes rather than in a pan.

I think this depends on what you are going to use the frosting for. My guess is that, by saying frosting, you are talking about buttercream or some other cake frosting, so I will run with that.
Honestly, you can, but it won't be quite the same. Here's why: Frosting is dependent on how well you can get the fat molecues filled with sugar and air. To achieve this, you need to whip the frosting very, very rapidly. Mixing frosting by hand will result in a less fluffy frosting that may or may not seperate on you.
Okay - that said!!! Another possibility is making a glaze or royal icing.

but you can make a chocolate ganache by melting bittersweet choc and heavy cream together and pour it over your cake for a beautiful rich icing.
You can make cream cheese frosting without a mixer. The secret is to let your cream cheese and butter come to room temp., and sift the powdered sugar. Use:
1 8-oz. package cream cheese at room temp.
1 stick unsalted butter at room temp.
1 teaspoon vanilla extract
1 1-lb. box of powdered sugar, sifted
In a bowl, with a wooden spoon, stir together well the cream cheese and butter. Stir in the vanilla. Stir in the powdered sugar, a cupful at a time, until of spreading consistency. Ice the cake and refrigerate. Refrigerate leftovers.

The absolutaly most simple is confectionary sugar with milk, mix them together with a spoon adding the milk to the sugar until you get it to the consistancy that you would like ( Thicker or not as creamy ).
