You sure can, although the will definitely have a 'tangy' taste to it. It could actually be quite good but perhaps a tad overwhelming. Add a bit more cheese than usual or some regular milk in addition to the buttermilk, it'll "soften" the tanginess.

Buttermilk is very low in fat. Cream is very high in fat. We use cream in cooking not because it's dairy, but because the fat content helps to make the flavour more flavourful; fat conveys flavour better than water does, which is why creamy soup is tastier than just meat stock.
Buttermilk is very acid and is not good for making pasta sauce with. Anyone who says it is doesn't know anything about food science. (One thing buttermilk is good at is marinading chicken in before you fry it, but that's a totally different recipe.)
If you don't have cream, then make something other than fettucine alfredo. It's as simple as that.
