Roly's fudge from Barnstaple in North Devon is amazing, all handmade stuff and some great flavours, eg strawberries and cream, Baileys, maple and walnut, whiskey and ginger and their vanilla one is amazing.

Checking on their website they have shops mostly in the westcountry, one in Brighton and a couple in the North - also surprisingly one in USA. I also saw their fudge on sale in Harrods.

Seroogy's in DePere, WI or Wilmar's in Appleton, WI. You can't get better fudge than from the dairy state! They ship all over the place and it's delicious and not as expensive as other places. I'd recommend the almond meltaways from Seroogy's, too!
