dont suprise her, just take her out for dinner, my cousin and i planned a surprise b-day party for my 50yrold aunt, she passed out and hit her head on the ground when we suprised her.
For my mom's party we wrote a "radio play" called "The Mystery of the Missing Mom". We did it like an old-fashioned radio mystery, with sound effects provided by the guests and us five kids playing the parts. Because we did it radio play style, no-one had to memorize lines, we just read them in front of an old fashioned microphone. We made fake "commercials" for our family's favorite products and restaurants, and included a lot of inside jokes. It was a huge hit and my mom just laughed and laughed. If your family and friends are creative at all, they'd probably enjoy the challenge.
