Having a barbecue with ribs and chicken. Then go for a Bar hop have a good friend,relative or parents take you for the occasion meet the lime life bands or exotic dancers if its OK to call it that or maybe even one of the supper clubs at the lakes
Some local bowling alleys have something called "ROCK AND BOWL" on Friday or Saturday nights. I've been to a few and had a blast. Most have packages you can purchase which sometimes include food.
Book a nail salon. Let the girls get pedicure and manicure. Then they can go to dinner and a movie.
I have always favored themed partys. Some of my faves are...
Vegas Night... Either rent a hall/bar or in a basement . Set up tables for black jack, dice , poker..etc... have them dress up like they are going out for a night on the town. Serve drinks like ( if they are are over 21) Champange and cranberry juice cocktails. in martini or flutes. Have some misc prizes for winners with most chips at end like a walkman, or gift certificate. And if they want to go paint the town .. they are already dressed up!
How about a fondue party. You can buy nice fondue pots for under 20 dollars. I got mine on clearance at Marshalls. You can also find tons of used ones in thrift shops. You could do a spagetti fondue, with jumbo shells, ham., mini garlic bread cubes to dunk in the spagetti sauce, or a traditional cheese fondue. For dessert you could dunk strawberries, bananas in chocolate fondue. My 12 year old has fondue parties all the time, everyone loves it.
Have her friends and family over for dinner and cake at home, and then hit a club or bar for a round of drinks.
My 21st is going to be in May and I already have plans. Me, my boyfriend, and a bunch of family and mutual friends are going to go out to dinner then go bar-hopping in my hometown (it's a pretty big city so we shouldn't get bored). At first we were all going to go on a cruise but my boyfriend and I (not to mention most of our friends) are tight on money and wouldn't be able to save up enough. The next idea was a weekend-trip to Destin, FL. and bar-hop there and hang out on the beach. It wouldn't have cost as much as a cruise, but it still would have been too much. Plus, most of us would be drunk in an area we don't know too well and that's never safe. We decided to go to dinner and go bar-hopping here because it doesn't require saving up too much money (we know about the cost of getting into places around here, and sometimes if you tell them you're celebrating your 21st birthday, they'll let you in free) and we already know our way around town so we shouldn't get into too much trouble (wink, wink). The best advice I can give you is to make sure you have designated drivers (I'll have about three or four) and keep the celebration as safe and as fun as possible.
If the birthday girl is not too keen on the whole bar and drinking thing (or if you live out in the boondocks and have nothing to do where you are now), just have a party at someone's house or rent a place to house however many people will be attending. My family is really wild and crazy so they won't care about seeing me drunk, but if her family is conservative, a good idea would be to get a surprise party together and when the guest-of-honor walks in holler "surprise" and begin playing songs from the year she was born (year being 1986 of course). Make sure there are plenty of finger foods for the guests and a big birthday cake for the birthday girl. You could even get some trivia together from 1986 (or to make it a little more fair, from the 90's) and play some trivia!

Maybe have a party at your local sports bar. somewhere that you can order food or maybe even have a section for a party of 15.
Or do you have a Chi chi's or a Red Lobster in your town? chi chi's is good for doing birthday parties. They even have a bar where you can purchase her first drink. Oh, Chi Chi's by the way, is a Mexican eating place. So in case you don't have that one, try any Mexican place. The Mexicans are known for making a big stink about birthdays or parties of any kind. Especially, adult age parties.
May I suggest, a Strawberry Daiquiri or Pi?a colada for her first alcoholic beverage. I don't know your relation to the birthday person, but they also have a drink called "Sex on the beach". If your like the parent maybe that would be kinda weird to get.
