I'm taking him to a really smart asian restaurant/club.
I thought of preordering the food and getting then to place the sushi in the shape of '21'.
I need more ideas. what can I get the waiters to do, to surprise him?

Preordering sushi might not be the best idea. Sushi is best served extra fresh.
I think it would be interesting if you got him a present and, when you order an appetizer, you have the waiter bring a little gift box on a plate instead!
What you CAN preorder is the alcohol. Have the restaurant prep a chilled bottle of champagne and put it tableside for your arrival.
