Hmmm...I am sure that there are rare instances of this happening, although I've never heard of it. (although I'm sure there are more who have fallen sick from it) When indulging in this sort of thing, as with beef carpaccio, sashimi, and some sushi's, you just want to go to a reputable establishment or make it with the freshest ingredients possible. I make my own sushi and beef carpaccio, as well as enjoy them out quite often, and have never had any problems at all.
In the past, French restaurants, used to present the cut of meat for you to inspect before they made the dish of Beef Tartare. I don't know if that has changed, but it certainly seems like a good practice to let the customer see it, (though I would never eat raw meat myself.) There have been instances where people became ill from eating sushi because the fish or meat was tainted. Again, you need to be sure that the restaurant maintains the highest standards. Nowadays, I'm very skeptical about any meat that isn't well-cooked.
Not that I've heard of. Raw beef is something that we eat in Wisconsin. You put it on rye bread, with salt, pepper and raw onion. I've been away for awhile, so I don't remember if it's ground chuck that is used for it, or ground round. I never ate it growing up, thought it was pretty disgusting. Tried it as an adult, and didn't find it that bad.
Only if you eat it with carpers.
