It is very healthy. You may want to keep an eye on the sugar counts in the fruit and trail mix though.

breakfast cereral should be oatmeal, smart start cold cereral, or cherrios, shredded wheat and milk eat some fruit also
snacks could be peanuts, popcorn, tea or water
lunch should be healthy food, tuna sanwich with wheat bread drink milk or juices........dinner could be chicken breast no skin, yam and veggies dinner salad...
The snack sounds healthy, but too big. Cereal completely depends on the type. The same goes for the sandwich. Backed chips are still chips. They aren't a great option. You could replace them with a vegetable.
I would go with either the banana with peanut butter OR trail mix. If you still feel hungry, eat some baby carrots and celery with your banana (they also taste good with peanut butter). Or add some high fiber dried cereal and pretzels to your trail mix. Frosted mini wheats, cheerios, and raisin bran are good cereal options. I also like to have granola bars for snacks. Make sure to look at labels though, some may be low in fat and calories but they may have very little fiber too and lots of sugar. Curves brand is very good especially the 100 calories bars with 5 grams of fiber, they actually taste great!
switch the trail mix to yogurt
peanut butter has protein that can't be utilized without dairy (the proteins compliment each other)
bananas have prebiotics which the probiotics in the yogurt feed off of.
This is a perfect trifecta of complimentary foods and a total power snack!!
