You're very used to the taste of processed food, so to start, make things from scratch. Make your own mac & cheese with real cream and cheddar, spaghetti sauce from vegetables, not a jar. You might take sandwiches off the menu for a little while, unless you're making them with good whole-grain bread and meat that hasn't been treated with preservatives.
Also, try at least one new vegetable each week, prepared differently each time. You might also try experimenting by cooking dinner for the family. (take it slow, just make one thing at a time if you aren't used to doing this)

I think you should do two things. First, keep a food journal for a couple of weeks. Write down everything you eat so you can get an idea of what your diet is really like. Then make a list of the foods you like that you know are unhealthy so you can start avoiding them. Some simple changes will help you a lot.
As you get older a lot of the food you used to hate becomes more appetizing. If you start trying new things you will be surprised at how much your tastes have changed.
Try mixing new stuff into the stuff you like.
Add veggies to your pasta.
Add tomatoes, lettuce, ect on to your sandwich.
If your Parents don't know about any of it tell them and they will do something if they know ask if you can go to a doctor and he or she will help you
