Obviously, any produce you have just plucked from the plant yourself. Surprisingly enough, in the grocery store the freezer case may offer fresher choices-foods are frozen within hours of harvest, as opposed to "fresh" produce that just spent several days on a truck.
But one thing that epitomizes "fresh" to me-a fish pulled out of the water, immediately cleaned and cooked yards away from the source. No other fish compares. I have had brook and rainbow trout, catfish, bass and sand shark prepared this way and the flavor screams "fresh".
fresh - or "produce" means that is is not treated, preserved, pickled, frozen, salted, in a jar, dried etc.
it can be refrigerated or packaged, but it is left in natural state.
eggs,
milk - (although pasteurised this is to kill germs, not lengthen shelf life)
fresh meat or fish - not cured or dried or smoked
vegetables, fruit, herbs, nuts, mushrooms.
also you can get fresh cheese (not aged) such as mozzerella, feta, goats cheese
fresh baked goods, bread, pies, pastry, cakes (made out of any of the fresh ingriedients above)
flour, pasta.
fresh does not always mean raw. it can be cooked if made our of above produce
