In Southeast Missouri it's 3.99. But you have to order at least 7.00 here to have anything delivered by them. With a medium cheese pizza (8.99) and the dessert (3.99) plus tax = 13.89.
I would tip like $2.00, depends on the distance the driver had to drive.
When I worked for Domino's Pizza, tips ranged usually from 1.00 to upwards of $5.00
It's true that good tippers would get faster service. Everybody remembers the good tipping places and would bargin to get the delivery.
