There are a number of recipes available. This works for me. Modify quantities to suit your taste.

6 cups freshly brewed extra strong coffee or expresso
4 ounces semisweet baking chocolate (4, 1 ounce squares)
1 1/2 cups milk, scalded
1 teaspoon peppermint extract

I use 1/3 cup of good freshly ground coffee to 6 measuring cups of water.
Pour brewed coffee into pan and add chocolate squares.
Heat, stirring regularly until chocolate is melted and mixed into coffee.
Stir in milk and extract.
Serve.

Depending on your taste, you may want less milk, more chocolate or more peppermint extract.

I have a friend who uses a similar recipe except he insists the best flavor comes from expresso coffee and vanilla syrup vice extract.
