I'm not sure about the chemical reactions that take place, but I would say yes. The sugar plus caffeine content of a soda might stress the heart more than normally. Caffeine is a narcotic, addictive, in otherwords, plus it is a stimulant, making your hear rate speed up, nervous system speed up, etc. I would recommend for someone with heart disease to lay off of the soda.

The caffeine, sugar and carbonation will definitely speed up your system and cause your blood pressure to go up.
