I was on an airplane ride to Armenia once and the lady asked me if I wanted tea, instead she brings me Armenian coffee which is almost twice as strong as our espressos. So not to be rude, I drank it, but woah!

It's never happened to me with coffee/tea, but it's happened with Sprite/Water. I ordered Sprite and didn't notice until the bill came (the Sprite was not on it). The waitress confirmed it. I just had Water with dinner. I must have forgotten! lol
I've never actually drank something without realizing what it was (except in college).
On a few occasions, I've ordered one thing and gotten another at a restaurant. I like coffee and tea, so I would typically just drink it without complaining. But I've had it happen where I order a regular (non-"diet") drink, like Coke, and the server has brought me a diet beverage. I'm one of those people who CAN taste the difference, and I've politely told the server about the mistake. Not a big deal; wait staff are human, too.
