Cream rises to the top and is skimmed off. That's why milk with all the cream removed is called skim milk.

I'm not really sure how the manufacturers do it but cream floats to the top of the milk. In the old days when people bought milk it had a couple of inches of cream on the top. You can spoon it right off.
