I made cappuccino and all the grains went into cup? Help please?

i just bought this cappuccino maker...but its not new and i dont have the instructions..so i made it and as soon as the water started comming out so did all the coffee grains into my cup...can someone help me find out why this is? oh am i suppose to put the steamer right into the milk or just above it?

you probably didnt use enough force when pressing the coffee into a puck...you have to use 40 lbs pressure at least but no more than 50lbs.....also...you put the steamer into the milk
