Brew coffee a little stronger than normal. refrigerate till cold.

Mix equal amounts of chilled coffee and chocolate milk (or Hershey's syrup and regular milk) and pour over a glass of ice cubes.

If you prefer blended, use one cup ice to one cup coffee/milk mixture and blend on high in blender until smooth.
