Question: I'm already a Vegetarian (well i'm almost there ^_^) but i want to go totally Vegan now, can you help me?
How do i plan my diet out?

Becoming vegan is the best decision I ever made. I'm sure if you take the plunge you will love it. I won't post any more links since Karen already posted a whole bunch of great links. But the main tips I can give you are:

Stay in contact with other vegans for moral support. If you don't know any vegans (in real life) YA is a great source of information and moral support.

Keep informed. Keep reading and learning, there's so much info out there. Stay up to date by subscribing to newsletters, veganoutreach newsletter is good.

Learn how too cook and improvise with different ingredients. Buy or borrow some good cook books so you don't get bored. One of my faves is 125 Best Vegan Recipe's by Maxine Effinson.

Make sure you eat a balanced diet including beans, nuts, legumes, fruits, veggies, leafy greens, tofu, soy milk & good whole grain breads. You don't need supplements if you're eating a balanced vegan diet. But I don't think they don't hurt if your concerned.

vegcooking.com has a vegan menu section that will have planned out menus for you, as well as tons of veg recipes.

vegweb.com is by far the best vegan recipe site, all the recipes are posted by users and are also rated by users. I love it.

I've been vegan for about 7 or 8 years, it can be a challenge at first, but its super easy once you get the hang of it.

its hard for me to write out everything you should do, i mean, you can google and find lots of ways to go vegan, and the nutrients you need.

i'd google for vegan iron sources, calcium sources, etc...

one of the best tips with going vegan is make sure you get your protein and try to eat lots of fruits veggies, whole grains, and legumes. don't make the mistake of eating too many carbs, a lot of new veg*ns do that because they dont know what to eat.

Since you are a vegetarian, you know that they eat only vegies, salads, no meat and dairy products. Vegans eat vegies and salads and no dairy products. Note: That vegans must supplement their diet with certain vitamins and this means that they eat more of certain foods than others. When practicing being a vegan, it's important to have a balance vitamin intake so your body does not get depleted of certain vitamins.

Be careful though, you will have to take a lot of food supplements, for example iron. You can get iron in green leafy vegetables, but nowhere near in the amount as you get it in red meat. If you don't take OBSESSIVE care about the amounts of minerals and metals in the stuff you eat, you'll become anemic.
