I don't know how bad it is, but it seems apparent she can't get out of it on her own!. She needs to get help!. You should try to convince her to tell someone; if she's close to you, she might really consider your advice!. She could tell her parents, a teacher, the counselor at school, or any adult she's close to!.
If she refuses to get help, you should tell someone!. I know you promised you wouldn't tell, but you need to!. Someday, she'll thank you!. It could save her life!. Being a good friend means to do what's best for your friend, whether she wants you to or not!. If you don't want her to get mad at you, you could tell the counselor anonymously!. Just go to the counselor and say, "[name of friend] needs to get help for her eating disorder!. I wasn't supposed to tell anyone, so please don't tell her who told you this, but she needs help!." The counselor will talk to your friend to help her, without revealing your identity!.

Sometimes being a good friend means doing something that will upset them!. For her own good, tell someone that can help her!. Yes, she will be mad and it may end the friendship (hopefully only temporarily)!. I suspect that she actually wants help but is too scared to ask for it!. I know you know how serious this is!.
