Switch to a low-fat raw vegan diet. Before this year I was a vegan, favoring grains, beans, and nuts. I've been eating mostly fruit along with leafy green vegetables for the past month, and the texture of my skin is completely different, every inch very soft and smooth. I can't stop feeling my nose! I haven't gotten any new breakouts, my backne has totally cleared up, and the acne on my face and eczema on my arms is about half of what it used to be. I expect that if I keep this up, these blemishes will be gone after several more months.
Even if you don't want to make such a radical change, you could try eating more fruit, and limiting the percentage of your calories that come from fat (below 10 or 15 percent ideally): http://www.raw-food-health.net/High-Fat-Diets.html.

I use an organic bar of soap with Tea tree oil in it. You can easily make your own with a soap-kit from a craft store and some tea tree oil, which I've been able to find at Trader Joes for about $7 a bottle. Its a very small bottle but goes a very long way and tea tree oil is great for your skin. Lather the soap on and use a facial scrubbing pad (like a brilo pad but a softer version specifically for your face) in a circular motion all over your face, pay special attention to creases like the sides of your nose and any problem areas. Rinse well and use a gentle moisturizer, I use Aveeno Active Naturals daily moisturizer but whatever works best.
I used to break out a lot but havent had a big breakout since i started using the soap, of course i still get the odd pimple here and there but almost always just one at a time and easily covered up.
Try completely cutting out dairy for a week and see of that helps. Dairy is typically a culprit in bad skin - I had sucky acne until I got on the Pill, and then it wasn't as bad, but when I went vegan my skin completely cleared up. I haven't had skin this clear since I was a child. =D
Just try it, and if it doesn't work, maybe you can find a dermatologist to help you out. :)
