Answers:
I use fat free American cheese, spray the bread with butter flavored no calorie cooking spray and grill it in my foreman grill. Its fast, tastes yummy, and there are no extra calories from butter or oil. :)

Hmm..the only way I know of a healthy one would be to talke away the butter, cheese, and just eat a pc of toast. Too many calories in a grilled cheese!

Substitute lettuce for the cheese.

The only way I would know would be to cut out the butter totally. And to do that you would have to make it with a ho-bo pie iron used for camping. I think typically you could find reduced calorie cheese and bread, but your problem is going to be the butter on the outside of the bread. By eliminating the butter altogether...I think that would be your best bet.

I like to grate part skim mozzarella and a little bit of well aged cheddar for taste on sprouted grain (Ezekiel) bread. Brush olive oil on the outside of the bread slices so they brown nicely on the fry pan.

Spray the pan with Parkay spray butter. No fat, no cals. Use reduced fat cheese and wheat bread. That's about as healthy as you're going to get a grilled cheese. It's good to throw on a slice of tomato and some lettuce, that way you're at least getting a serving of veggies.

Find the healthiest whole grain bread you can find, I like honey wheatberry, the average slice of bread is approx 20 carbs but you can find some as low as 12 to 15. Use cheese that is not processed, provalone and cheader together are a good choice. Now for the ingredient that wont make sence to you but it is best, use unsalted butter as the lubricant. Small portion of butter presents no health danger is easier for digestion, has great flavor, and browns best. You could use a spreads or margarine but this is "hydrogelated" vegtable oil, they take a liquid and make it a solid, not good. Butter is naturally made solid by agitation no additives.

Finally when ever you can find organic products for the ultimate in healthy eating buy them, however be prepared to pay a little more, and some breads may not tase as good.

Nobody has ever died from eating a grilled cheese sandwich. Make it with wheat bread if you want, maybe use as little butter as possible and add lettuce or tomato to it if you like the taste, but don't go nuts. Just make it and enjoy it - life is too short to wonder if one grilled cheese sandwich will have a serious impact on your health. It won't, so live a little. Use two slices of cheese, drink a glass of whole milk with it. Cheese, butter, white bread - they aren't AS healthy as some other alternatives, but they are far from UN-healthy as well.

I use I Can't Believe It's Not Butter on the bread and make it on the George Foreman grill. It's the best.
