Not from sugar and cocoa powder is unlikley. Generally, these maggots are found in grain products like brown rice, cornmeal, etc. The more refined the grain, the less likely they are to be a culprit. Dry pet food can also bring them into your home.
Toss out all the grains products and stuff like pasta that are opened. Spray with a pyrethrin. When you bring any rice or polenta or corn meal home, put it in the freezer for at least a couple of days. That will kill any grain beetle eggs that might be in them. You can store them in the freezer if you want. If not, transfer them to a sealed container, like rubbermaid or tupperware.
You should not have problems after that.

As stated, I think the "tiny white worms" are actually Maggots.
It sounds like either flies, moths, or some other bug has laid eggs in your sugary foods.
Sugar, Flower, Cocoa Powder, anything ground should be kept in an air-tight container, like the old Jars that Grandma used to have, or Tupperware.
You should throw out ALL the foods you have that are not in airtight containers. Unfortunately, you're seeing them in these one or two items but the eggs could be very small and in anything, and you might eat them accidently... (guess it would add protein).
You are now seeing the Maggots becuase they've eaten and are looking for a place to go and metamorphose into whatever they will become.
Good luck with that... infestations like this are hard to erradicate.
usually tiny worms come from moist soil, the Earth's top layer, rain makes them venture into unknown territories as well, never seen them in my chocolate milk or sugar, must check the kool aid next time.
