alcoholism is an obsolete term. The old term was vague and ill-defined, sometimes refering to anybody who consumes, otherwise indicating drinking to such an extent it produces problems for the drinker or to those around him (literally and metaphorically!)
Today we use terms like alcohol-related problems, problem drinking, alcohol use/misuse/abuse and dependence. Most of these are well defined and can be used as diagnostic entities.
Yes "traditionally" doctors made judgements, usuall based on their own template of how much is ok. Today its a complex assesment taking into consideration amount, type, frequency, where and with whom drinking takes place. Attention is paid to medical or social problems that have arisen from this activity. So there!
