You know, I see those on vacation all the time. Do you have a tourist-type store? They'd have them there! You can get one with your name on it unless you have an unusual name.

Souvenir shops often have them (especially cute ones); places that sell liquor may have them; places that sell glassware may have them...

Souvenir shops is a good idea, but they are often called "toothpick holders." High schools and colleges sell these "toothpick holders" in their gift shops and stores.
