Answers:
On the rocks, straight up, dirty (as in a martini).

'on the rocks' is always classy...it means with ice.

If anyone at the bar I work at orders salt and lime with a tequila shot, we call it "training wheels." I didn't have a clue what "muddling" (mashing the hell out of lemons or limes, over ice in a shaker) was when I started, suppose that could be slang. Martinis with very little vermouth are considered "dry." Throw in some olive juice and it's "dirty." Salt or sugar around the rim of a glass (rub a lemon or lime wedge around it so the salt/sugar sticks) and you've got a "rimmer."

Don't know if you'd consider types of glassware as slang, but there are loads.. Tumblers/buckets, hurricane/highball, snifters, various types of stemware, pounders and pints...

Oh, and my personal favorite, eighty-sixed (get the hell out, don't come back!) So yeah, there's tons of different bar slang. Depends on where you drink, too. If you walk into a biker bar and ask for a cosmo up with a rimmer you'd probably get beat up, or at the very least laughed at.
