Its goes through a second chiller unit usually under the bar where the glasses are kept. This is done so the same barrel can supply more than one pump!

its just cooled to a lower temperature in the pipes...................mines a pint,

It doesn't stay extra cold when exposed to room temperature being in your glass....so drink it up nice and quickly!

It is cooler simply by being passed through the cooler twice rather than once which normal temparature beers are......the difference is only a degree or two but it does give the lager that fresher cooler taste......although after a few who notices anyway?!

They have a 2nd cooler as the beer comes out. You can see on the Pump the water.

There is only one way to keep it extra cold and that is to drink it within 1 minute, after that it starts to warm up.
Just thinking about it is giving me an urge for one, but I have another 5 hours before I finish work yet!

Extra cold means that it is stored just barely above freezing temperatures, like at 33 degrees F.

the question you are asking is somewhat vague. if they are -serving- the beer as "extra cold" then they may run it through a second chiller as it comes through the tap (this is more common in the US than in Europe where beer is served around 50F).

another way of answering the question is with regard to "lagering" beer itself. there can be two types of fermentation- ale and lager. lagering is typically done at much colder temperatures with a special yeast (lager yeast) which is more hospitable in colder temperatures. so, if they are saying "extra cold lager on tap" it might not be how it is served but how it was made.
