You always tell the truth to your doctor because they can't help and a private physician will help you help yourself. It depends on insurance too because you need to be truthful to yourself.
I have a very nice doctor and would never lie when it comes to concerns over my health. You only get one life.
Of course, its your Dr.. they will find out eventually if you don′t.. So be Honest...

If the drinking is relevant to the problem then you should be very very honest. So for a general physical...yes. Remember, you need all the feedback the doc can give to stay healthy.
