First would be "cork" taint. Some corks contain a chemical called TCA and it makes wine taste musty or moldy. Next is high temperature and/or light which cause unpleasant chemicals to form. Another is temperature variations. Even at relatively low temperatures (say 55 - 65), if the temperature cycles alot, the wine can be damaged. Lastly, if there is a musty smell in the environment, it can invade the wine over time.

Temperature fluctuation during storage.
Contaminated/dirty bottle at the time of corking.
Bad cork seal (excess oxidation)

The above are a few factors that can "punk" a good bottle of wine.

In regards to telling if a bottle is bad, it all comes down to taste.
